Student Grade - For Summer Term classes, list ENTERING GRADE for upcoming Fall. Other classes list CURRENT GRADE.
ONLINE PUBLICATION: Student films and behind-the-scenes footage from my child's Summer Film Camp will be compiled and uploaded to a private, online video album established for his/her individual camp. My child and I will receive a password to the album, as will the other parents and students in the camp. We will be able to download from the album, as will all members of the class. I will encourage my child to share the films in a way that is respectful and appropriate.
BEHAVIOR: I will encourage my child to behave in a way that is conducive to a fun and healthy learning environment. In the event that my child is being overly disruptive, he or she will be removed from the classroom. If the behavior persists, I will be notified and asked to help address the issue. If the behavior does not improve, my child may be asked not to return to class, and my tuition will not be refunded. If my child might have difficulty meeting these expectations for any reason, I will detail this info in the Additional Information box below, so that appropriate responses or strategies may be discussed prior to the beginning of camp.
ALL AGES: My child will NOT be released for any reason during the camp session itself (e.g. breaks, lunch). Departures for medical appointments etc. must be arranged ahead of time with staff. I may grant permission for another adult to pick up my child but must do so ahead of time in writing (please enter authorization information below). Telephone authorization will not be accepted. Unauthorized walk-ins will not be accommodated. YOUNGER THAN 13 (12 and younger): I (or an authorized individual) am required to drop off my child and pick up my child each day in person, and will furnish ID and signature upon staff request. AGES 13+: I have the option of authorizing my child to depart independently at the end of the camp day without my presence and signature. I am solely responsible for my child's behavior and actions once they leave the building. By checking the option below, I authorize my child, who is aged 13 or older, to leave independently.
I grant permission for my child to arrive/depart independently (after day one of camp). AVAILABLE OPTION FOR STUDENTS 13 AND OLDER ONLY.
I do not grant permission for my child to arrive/depart independently and understand that I am required to drop off my child and pick up my child each day in person (with ID). REQUIRED FOR STUDENTS 12 AND YOUNGER.
The following individual(s) are authorized to pick my child up:
